Copyright Notice Procedure

At NY PDF Forms, we are committed to protecting copyright and upholding the intellectual property rights of others. We understand the importance of these rights in fostering creativity and innovation. Our Copyright Notice Procedure is designed to address concerns related to copyright infringement swiftly and effectively. If you believe that your work has been copied in a way that constitutes copyright infringement, we are here to help.

Submitting a Copyright Notice

To file a copyright notice, please provide us with a written notification containing the following information:

  • A physical or electronic signature of the person authorized to act on behalf of the owner of the copyright interest.
  • A description of the copyrighted work that you claim has been infringed.
  • A description of where the material that you claim is infringing is located on our site, with enough detail to allow us to locate it on the website.
  • Your address, telephone number, and email address, so that we can contact you about your complaint.
  • A statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law.
  • A statement by you, made under penalty of perjury, that the above information in your notice is accurate and that you are the copyright owner or authorized to act on the copyright owner’s behalf.

Please send all copyright notices to dmca@nypdfforms.com. Upon receipt of a valid complaint, we will take the necessary actions to address the issue, which may include removing the allegedly infringing material from the website.

Processing of Notices

All notices received will be reviewed in a timely manner. Our response will involve examining the details of the complaint, verifying the information provided, and taking appropriate action in accordance with copyright law and our policies. We strive to resolve any issues efficiently, keeping open communication with the parties involved.

Counter-Notice Procedure

If you believe that your content has been incorrectly identified as infringing, you have the right to send us a counter-notice. Your counter-notice must include:

  • Your physical or electronic signature.
  • A description of the content that has been removed and the location at which the content appeared before it was removed.
  • A statement under penalty of perjury that you have a good faith belief that the content was removed or disabled as a result of mistake or misidentification of the content in question.
  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court in your jurisdiction, and that you will accept service of process from the person who provided notification of the alleged infringement.

The same email address, dmca@nypdfforms.com, should be used for submitting counter-notices. We will inform the complaining party of your counter-notice, and if your counter-claim is valid, we may restore the content in question.
